Practice Policies

Federal regulation restricts the prescription of any controlled substances virtually.

Financial Terms

Private Pay and some commercial insurance are accepted. All fees must be paid to Above and Beyond Psychiatry PLLC at the time of service. Appointments will be rescheduled if financial obligations are not met.

Billing Fees

Insurance panels are still in progress, and as such, only private payment methods are acceptable at this time.

1. $200 for new patient appointments.

2. $125 per follow-up appointment.

Payment can be made with cash Pay only. Insurance is still in progress

While all fees are due at the time of service, any unpaid balance should be paid within 30 days of receiving a billing statement from us. Patients will also be charged for treatment codes not covered by your insurance. Collection services will follow if fees continue to remain unpaid. We reserve the right to provide our services further if terms are not abided by.

Practice:

My primary practice goal is to provide prompt, individualized, reliable, and compassionate mental health care to improve the mental symptoms of each individual through the integration of psychopharmacology, mental wellness education, and psychotherapy.

New Patients:

During your initial visit, which will take approximately 1 hour, a thorough evaluation will be done, followed by a diagnosis, and an individualized treatment plan will be created. The initial plan will include a 2-4 weeks follow-up or as-needed appointment.

Follow-up visits.

Follow-up visits will last for 30-45 minutes, depending on the patient’s situation. These appointments will be used to re-evaluate treatments, educate, and manage medication. Further follow-up will be determined and planned, depending on the patient’s situation, with the main goal of improving and stabilizing the patient. For a long-term and stable patient, follow-up appointments will be scheduled within 90days or as needed to maintain a safe and effective treatment.

Refills:

Medication refills are planned to coincide with the appointment, but that notwithstanding, should a refill be required outside of our scheduled appointment, contact your pharmacy as soon as possible and allow 5 BUSINESS DAYS for it to be processed. Keep an eye on the number of your medications to avoid any rush. I am legally required to ensure that you are in inactive treatment if I am prescribing medication therefore, I will not authorize a refill if there is no upcoming appointment scheduled. Only a one-time over-the-phone refill might be allowed.

Cancellation Policy

I understand things can change, and you may want to cancel your appointment. All I ask from you is to provide our office with a notice 24 hours before your scheduled appointment. Notifying us even days in advance will allow me to see someone else who needs to be seen since we do not double-book your appointment time with any other patient.

In the event that you are unable to provide a cancellation notice before 24 hours of your scheduled appointment, you will be charged $50 if you were scheduled for a 30-minute appointment and $100 for a missed 60-minute scheduled appointment. This fee is not covered by your insurance. I might also stop providing services to you if the recurrent cancellation pattern is noticed. Should I not also be able to provide service to you as scheduled, you will be adequately notified and will happily reschedule your appointment.

Confidentiality:

I am compliant with the Health Insurance Portability and Accountability Act (HIPAA), a federal law that provides privacy protections and patient rights with regard to personal health care information (PHI). Your information will be strictly confidential before, during, and after services. For the most part, your information will only be released with your consent or your guardian’s if applicable. Some exceptions include if you pose a danger to yourself or others, if child abuse, exploitation, or neglect is suspected, it is required by law to release confidential information. In some instances, also, the court may subpoena treatment records. that notwithstanding, any release of confidential information will be made known to you. 

Distinguishing between an emergency, urgent, and non-urgent request:

If you feel suicidal or homicidal or a medical emergency, you should call 911 or go to the closest emergency room. If you need a consultation outside of an appointment due to an increase in symptoms or medication side effects, give us a call and ask for my availability.

Disability Claims

If you are requesting assistance in the completion of a disability claim, please make a note of the following:

We do not process paperwork or evaluate for Social Security Disability! ****

We can only report symptoms and response to treatment to the company that handles your disability insurance. 

Your Consent;

By reading this disclosure and agreeing to continue with treatment this will serve as consent for treatment. By this, you authorize your provider and to release information to your insurance carrier(s) listed and be paid directly by the insurance carrier(s) for services billed. You acknowledge that you are responsible for all charges not paid by my insurance companies including; copays, coinsurance, deductibles, insurance plan refusal to pay for failure to obtain authorization, and missed and late cancellation fees. If it becomes necessary to effect collections of any amount owed, the undersigned agrees to pay all costs and expenses, including reasonable attorney fees.
